Referrals are welcome

Who can make a referral?

A referral can come from the participant or from someone supporting them. Where another person refers, we will confirm the participant’s consent or the referrer’s authority before progressing.

A participant

You can refer yourself, ask questions first or have someone you trust help with the referral.

Family, nominee or advocate

A family member, carer, nominee, advocate or other trusted person can contact us with the appropriate consent or authority.

A coordinator or professional

Support coordinators, recovery coaches, LACs, health professionals, hospitals, allied health teams and community partners can refer.

A referral does not remove choice and control

The participant can ask questions, compare providers, involve a trusted person and decide whether HelpAlong is the right fit. Sending this form does not create an obligation to proceed.

Before you begin

Information that helps us respond

You do not need every detail before contacting us. The following information helps our Intake team understand the request and prepare for the first conversation.

  • Participant name, suburb or preferred service location and safe contact details.
  • The support being requested and the participant’s main goals and preferences.
  • Relevant NDIS funding category, plan dates and how the plan is managed, if known.
  • Preferred commencement timeframe and whether the request is time-sensitive.
  • Communication, accessibility, cultural or compatibility considerations for the first conversation.
  • The referrer’s relationship to the participant and confirmation that contact is authorised.

A practical intake pathway

How the referral process works

We listen first, explain the available options and only progress when the participant or authorised representative is comfortable with the next step.

    Complete the online form or contact Intake by phone or email with the basic referral information.

    We make contact

    Our team reviews the request and contacts the referrer, participant or authorised representative to clarify needs.

  3. 03

    We discuss fit and capacity

    Together, we consider goals, funding, location, timing, compatibility, risks and the supports currently available.

  4. 04

    Agree on the next step

    This may include a meeting, assessment, property visit, secure information request, service proposal or another suitable pathway.

Initial review

We review the referral details, confirm the requested service and identify any information needed for a useful first discussion.

A conversation with the right person

We contact the nominated person using the details provided and involve the participant or their authorised representative appropriately.

Suitability and capacity check

We consider service scope, location, timing, worker capability, compatibility, funding and whether safe support can be arranged.

Clear next-step options

We explain what can happen next, what further information is required and whether another provider or pathway may be more suitable.
