Changing Providers and Planning Continuity

A practical checklist for changing NDIS providers safely.

Changing provider is a normal participant choice. A planned transition can protect safety, relationships and continuity.

Before making the change

  • Review notice and ending terms in the current service agreement.
  • Confirm the new provider’s capacity, start date, price and responsibilities.
  • Decide what information can be shared, with whom and by what secure method.
  • Plan worker introductions, equipment, medication, routines and emergency contacts.
  • Confirm who will manage gaps, final claims and records.

Home and living arrangements

If housing and support are connected, ask how changing the support provider may affect the tenancy or occupancy arrangement. The current SIL Practice Standards expect providers to support informed choice, manage conflicts and protect housing stability. Arrangements vary, so independent tenancy or advocacy advice may be useful.

Urgent safeguarding situations may require a different transition plan. Call 000 for immediate danger or contact the NDIS Commission for an independent concern.
